Congestive Heart Failure (CHF)
Congestive heart failure is a common cardiac problem in the US. After age 40, 1 in 5 people will develop some form of heart failure and the risk increases as we age. Depending upon the severity of the dysfunction, patients will have few symptoms to severe limitations to their activities of daily living, like dressing or bathing. Heart failure also increases the chances of a hospitalization and death. Lesser recognized “side effects” are depression and impaired cognitive (thinking) abilities either from the disease or the medications.
There are multiple causes of heart failure but most common are: coronary heart disease, cigarette smoking, high blood pressure, obesity, diabetes and valvular heart disease. Less common is viral or bacterial infection, pregnancy, substance abuse or inflammatory diseases like rheumatoid arthritis or lupus.
Treatment will be directed to the specific identified cause but in general all patients will be in cardiac rehab to improve functional capacity and quality of life. Weight control is also important to decrease the workload on the heart and improve blood pressure and diabetes control. Most patients will be on medications like spironolactone to help control electrolytes, diuretics like Lasix to help with fluid overload and an ACE/ARB like lisinopril or valsartan to help control blood pressure and improve renal function.
